A comprehensive hearing evaluation is an in-depth assessment of how well you hear and how different parts of your auditory system are functioning. It goes beyond a quick pass-or-refer screening by combining your health and communication history with several carefully selected tests.
How is a comprehensive evaluation different from a screening?
A hearing screening is designed to identify whether someone may need additional testing. It is usually brief and produces a pass-or-refer result.
A comprehensive hearing evaluation is diagnostic. It examines hearing across frequencies, speech understanding and—when appropriate—outer-ear, middle-ear, inner-ear and auditory-pathway function.

An online hearing check can be a useful awareness tool, but it cannot inspect the ears, control the testing environment or replace a diagnostic evaluation by a qualified professional.
Who should have a comprehensive hearing evaluation?
An evaluation may be appropriate when you:
- Frequently ask people to repeat themselves - Struggle to understand speech in restaurants or groups - Increase television, telephone or device volume - Notice gradual or one-sided hearing changes - Experience tinnitus - Have a history of significant noise exposure - Take medication that may affect hearing - Have dizziness or balance concerns - Did not pass a hearing screening - Are considering hearing aids - Have concerns about a child’s hearing or listening development
A physician may also request testing to investigate an ear condition or establish a baseline before medical treatment.
Before testing: the case history
The appointment generally begins with a conversation. The audiologist may ask about:
- When the hearing concern began - Whether it affects one or both ears - Situations in which communication is difficult - Tinnitus, dizziness, pressure, pain or drainage - Previous ear conditions, surgery or hearing tests - Family history of hearing loss - Occupational and recreational noise exposure - Current medications and relevant health conditions - Hearing-aid or assistive-device experience
These questions guide the test selection and help the audiologist identify symptoms that may require medical referral. Bring a current medication list and any previous hearing records if they are available.
Otoscopy: examining the ear canal
The audiologist uses an otoscope—a lighted instrument—to look at the outer ear, ear canal and visible portion of the eardrum.
Otoscopy may identify earwax, irritation, drainage, a foreign object or an appearance that warrants medical attention. It does not provide a complete medical diagnosis, but it helps determine whether testing can proceed safely and whether referral is appropriate.
Otoscopy should not be painful. Tell the audiologist if your ear is tender or if you recently had ear surgery or trauma.
Pure-tone testing
Pure-tone testing measures the softest tones you can detect at different pitches or frequencies. You may sit in a sound-treated room or booth and respond by pressing a button, raising a hand or giving another signal whenever you hear a tone—even if it is very soft.
### Air-conduction testing
Sounds are presented through headphones or insert earphones. This evaluates the entire hearing pathway from the outer ear through the middle and inner ear to the auditory system.
### Bone-conduction testing
A small vibrator is placed behind the ear or on the forehead. It sends sound vibrations through the skull, bypassing the outer and middle ears.
Comparing air- and bone-conduction results helps the audiologist determine whether a hearing loss is more consistent with:
- Conductive hearing loss: Sound transmission through the outer or middle ear is reduced. - Sensorineural hearing loss: The pattern involves the inner ear or auditory nerve. - Mixed hearing loss: Conductive and sensorineural components are both present.
Speech testing
Pure tones show the softest sounds you can detect, but everyday hearing depends heavily on speech understanding.
### Speech-recognition or speech-detection threshold
This measures the softest level at which you can recognize or detect speech. It also helps the audiologist check agreement between speech and pure-tone findings.
### Word-recognition testing
You listen to a recorded or spoken list of words at a controlled volume and repeat each word. The result describes how clearly you recognize speech under that test condition.
A strong score in a quiet test room does not guarantee easy conversation in a noisy restaurant. When real-world difficulty is greater than standard results suggest, the audiologist may consider additional testing or communication assessment.
### Speech-in-noise testing
Some clinics include a speech-in-noise test to examine understanding when speech competes with background sound. This can provide information closer to common daily listening challenges.
Speech-in-noise testing is valuable in many situations, but it is not included in every routine evaluation. Ask whether the clinic offers it if hearing in noise is your primary concern.
Tympanometry
Tympanometry evaluates eardrum movement and middle-ear function. A soft probe briefly changes air pressure in the ear canal while measuring the ear’s response.
It can help identify patterns associated with:
- Middle-ear fluid - Eardrum stiffness or reduced movement - Pressure-related middle-ear dysfunction - A possible opening in the eardrum - A blocked or nonfunctioning ear tube
You may feel pressure similar to changing altitude, but the test is quick and does not require you to respond to sound.
Acoustic reflex testing
Loud sounds normally trigger tiny middle-ear muscles to contract. Acoustic reflex testing measures whether that response occurs and at what sound level.
The results can add information about the middle ear and portions of the auditory pathway. Audiologists interpret them together with the rest of the test battery; a reflex result does not establish a diagnosis by itself.
Otoacoustic emissions
Otoacoustic emissions, commonly called OAEs, are faint sounds produced by healthy activity within the cochlea. A small probe placed in the ear canal presents sounds and records the inner ear’s response.
OAEs can provide objective information about cochlear function. They are often used in newborn hearing screening and pediatric assessment and may also be helpful for adults, monitoring or cases in which behavioral responses are limited.
OAEs do not replace the full evaluation. Middle-ear conditions, ear-canal blockage and other factors can affect the result.
Auditory brainstem response testing
Auditory brainstem response, or ABR, testing records electrical activity from the auditory nerve and brainstem after sound is presented. Small sensors are placed on the head, and earphones deliver clicks or tones.
ABR may be used to:
- Estimate hearing sensitivity when reliable behavioral responses are unavailable - Evaluate infants and young children - Investigate selected auditory-pathway concerns - Provide additional information when other test results do not agree
ABR is not required in every comprehensive adult evaluation. It is selected when the clinical question calls for it.
What is an audiogram?
The audiogram is the graph used to display pure-tone results. Frequencies, from low to high pitch, appear across the horizontal axis. Hearing levels, from softer to louder, appear down the vertical axis.
Symbols show the softest sounds heard by each ear through air and bone conduction. The audiologist may use terms such as mild, moderate, severe or profound to describe the degree of hearing loss across different frequencies.
The audiogram is important, but it is not the complete story. Speech results, middle-ear findings, symptoms, communication goals and daily environments all influence recommendations.
Understanding your results
At the end of the appointment, the audiologist should explain:
- Whether hearing is within the expected range or hearing loss is present - The type and degree of any hearing loss - Whether both ears show a similar pattern - How speech understanding was affected under the test conditions - Whether findings suggest a need for medical evaluation - Which communication or treatment options may be appropriate - When hearing should be rechecked
Ask for a copy of the results and written recommendations. If unfamiliar terms appear in the report, request a plain-language explanation.

Recommendations depend on the results and your goals. They may include:
- Monitoring hearing at a recommended interval - Medical evaluation by an ENT or another physician - Hearing-aid evaluation - Assistive listening technology - Communication strategies or auditory rehabilitation - Hearing protection - Tinnitus assessment or management - Vestibular testing - Cochlear-implant evaluation - Pediatric early-intervention or school coordination
Testing does not obligate you to purchase hearing aids. A comprehensive evaluation should support an informed decision.
How long does the appointment take?
The length varies by age, symptoms and tests needed. A routine adult diagnostic evaluation commonly takes about 30 to 60 minutes, while pediatric, balance, auditory-processing or implant evaluations can take longer or require more than one visit.
Ask the clinic how much time to reserve and whether a companion should attend. A trusted communication partner can help provide history, listen to recommendations and discuss shared communication challenges.
How to prepare
Before your appointment:
- Bring identification and insurance information - Bring your referral or medical order if required - List your medications and relevant medical history - Bring previous hearing-test results - Bring current hearing aids and accessories - Note the situations where hearing is most difficult - Write down your questions and goals
Avoid exposing yourself to unusually loud sound immediately before testing when possible. Temporary changes after intense noise may affect results.
Do not place objects or home remedies into the ears to clean them before the visit. Contact the clinic if you know you have significant earwax or active ear symptoms.
What does a comprehensive evaluation cost?
Cost depends on the tests, provider, facility and insurance plan. Diagnostic testing may be covered differently from routine screening, hearing-aid evaluation or device services.
Before the visit, ask:
- Is the audiologist and location in network? - Is a referral or prior authorization required? - Which tests are planned? - Will there be a facility fee? - What deductible, copayment or coinsurance applies? - Are hearing-aid services billed separately?
The clinic may provide an estimate, but the insurer makes the final coverage determination.
When not to wait for a routine evaluation
Seek prompt medical attention for sudden or rapidly worsening hearing loss, especially in one ear. Hearing loss after head or ear trauma, ear drainage with severe symptoms, or dizziness accompanied by weakness, facial drooping, difficulty speaking, fainting or a severe headache also warrants urgent evaluation.
Sudden hearing loss can feel like ear pressure or blockage and may be time-sensitive. Do not wait for an online test or routine appointment when hearing changes abruptly.

Is a comprehensive hearing evaluation painful?
No. Standard hearing tests are noninvasive. Tympanometry creates a brief pressure sensation, and some tests use louder sounds, but you should tell the audiologist if anything is uncomfortable.
Can I take a hearing test if I have tinnitus?
Yes. Audiologists routinely evaluate hearing in people with tinnitus. Tell the audiologist whether the tinnitus is one-sided, pulsatile, sudden or accompanied by other symptoms.
